Block Vision offers members a wide variety of participating providers from which to choose in seeking vision services. Our nationwide provider panel includes more than 18,000 provider locations, including both private practitioners and retail optical centers.

When choosing and accessing a participating provider, it is important to remember the following:

• Utilizing participating providers will help enable you to maximize your benefits coverage.

• You may choose your eye care provider when you are ready to use your vision benefits. It is not necessary to select a provider at the time of enrollment, and it is not necessary to obtain a referral form or to contact your Primary Care Physician.

• Once you have selected a participating provider, we encourage you to call Block Vision’s Member Services department at 800-879-6901 to ensure that the provider services your particular plan.

• When calling to schedule an appointment with a participating provider, please have your identification card readily available and identify yourself as a member enrolled in Block Vision’s program and advise the provider of the plan named on your identification card.

• Each family member may elect to receive services from a different participating provider.

• You may receive your eye examination and eyewear from different participating providers.

• On the day of the appointment, please remember to present your ID card.
